Shared decision-making periodic prostate-specific antigen (PSA)-based screening if aged 55-69

Counseling

For tobacco cessation

Advise users & provide behavioral interventions to stop using tobacco

For reducing unhealthy alcohol use

Advise users engaged in risky or hazardous drinking with brief behavioral counseling interventions to reduce unhealthy alcohol use

For STI risk reduction

All persons at increased risk for STIs

For healthy diet & physical activity

If age 18+ & at increased risk for cardiovascular disease (CVD) (e.g., high blood pressure, high cholesterol/lipids, or with mixed or multiple risk factors such as metabolic syndrome or an estimated 10-year CVD risk of 7.5% or greater)

Shared decision-making if age 18+ & not with increased CVD

For weight loss

If age 18+ & with body mass index (BMI) of 30 or higher (calculated as weight in kilograms divided by height in meters squared) be offered/referred to intensive, multicomponent behavioral interventions

For skin cancer prevention

Minimize exposure to ultraviolet (UV) radiation for persons with fair skin types to reduce their risk of skin cancer if age 24 or younger

Shared decision-making if older than 24

Prostate cancer

Shared decision-making periodic prostate-specific antigen (PSA)-based screening if aged 55-69

Medicatons

Tobacco cessation medicine

If nonpregnant & use tobacco

Pre-exposure prophylaxis (PrEP) for HIV

If at increased risk of HIV acquisition to decrease risk of acquiring HIV. Learn more here

Statin

If age 40-75 years & have 1 or more CVD risk factors (i.e. dyslipidemia, diabetes, hypertension, smoking) & an estimated 10-year risk of a cardiovascular event of 10% or greater. Learn more here

Aspirin

Individual-based decision to start low-dose aspirin use to prevent CVD if aged 40-59 years & have 10% or greater 10-year CVD risk

Procedures for cancer screen

Colorectal cancer

Each year, if age 45-75. Learn more here

​Lung cancer

Each year, if age 50-80 with ≥20-pack-year smoking history & currently smoke or quit in last 15 years. Learn more here

Other procedures 

Abdominal aortic aneurysm

If age 65-75 years & have ever smoked, 1-time screening with ultrasonography

Shared decision-making if age 65 & have never smoked

Dental visit

Each year (at least). Learn more here

Vision visit

Regular eye exam; get comprehensive dilated eye exam every 1-2 years if age 60+, are African American & older than 40, have family history of glaucoma; & every year if have diabetes or high blood pressure. Learn more here

Healthy lifestyle recommendations

Nutrition

  • Eating well will keep your body healthy. Learn more at MyPlate Plan at www.myplate.gov

  • Eat when hungry, stop when feeling satisfied

  • Do not skip meals, such as breakfast

  • Avoid drinks with sugar (soda, juice). Choose water instead

  • Have needs in getting food? Link to resources here

Physical activity

  • Do something that increases your heart rate (walking, running, sports, biking) for 1 hour Every Day. Learn more here

Sleep

  • Get 9 hours of sleep Each Night. Improve your sleep by

    • Limiting screen use before bed

    • Avoid caffeine, large meals, sugar, nicotine before bed

    • Limit naps during the day

    • Create a restful space to sleep

Safety

  • Always wear a seatbelt when driving in a car & a helmet & protective pads for playing sports, biking, skating, skiing, skateboarding

  • Always wear a life jacket when you do water sports. Learn how to swim!

  • Always wear sunscreen of 15 SPF or higher & a hat when outside to prevent from getting burned & skin cancer

  • Avoid distractions when driving. Never text or talk on the phone while driving

  • Do not ride in a car with someone who’s used alcohol or drugs

Dental

  • Brushing 2 times & flossing Every Day will keep gums healthy

  • See a Dentist 2 times Each Year for teeth cleaning & a check-up

  • Wear a mouthguard when playing sports

Vision

  • Way to keep your eyes healthy

    • Eat right to protect your sight

    • Wear glasses, as needed

    • Wear sunglasses year-round

Feelings

  • It’s okay to have up-and-down moods sometimes, but if you’re feeling sad most of the time, find someone to talk to

  • If you’re ever thinking about hurting yourself or someone else, or feeling you need help, find someone to talk to right away or call 988 (or link here)

Relationships

  • When in a sexual situation, “No” means NO. NO is okay.

  • No one should ever force or try to convince you to have sex

  • When having sex, correctly & consistently use condoms to protect against STIs/HIV & unintended pregnancy; use birth control to protect against unintended pregnancy

Self-care / mindfulness

  • Example self-care practices

    • Good hygiene

    • Eat healthy

    • See your doctor yearly, including regular screenings

    • Seek medical care when needed

    • Maintain regular sleep routine

    • Be active; spend time outside

    • Enjoy hobbies

    • Express gratitude

    • Treat yourself

    • Find purpose in your life
